____ Official GRE Test Scores - UCLA School of Public Health Code for ETS to report to SOPHAS: 4225
____ Official TOEFL or IELTS Scores (for international applicants only)
____ Writing Sample (for doctoral applicants applying to Community Health Sciences or Health Services) Submit writing sample in the additional questions section under "School Designations".
