How to Apply - MPH|HP

Applicants must complete the online UCLA Graduate Division application here. Select “Master of Public Health for Health Professionals MPH – Weekend Program” as the major and “Community Health Sciences” as the Department of Specialization.

Please submit the following materials:

1. Statement of Purpose
2. Personal Statement
3. Resume or Curriculum Vitae

Recommenders may upload letters to the application site or email them directly to ptoussaint@ph.ucla.edu. Letters should include letterhead (if possible) and the recommender’s signature. Letters should be from either a professional and/or academic source.

Official transcripts must be provided from each institution attended. Please request OFFICIAL electronic transcripts to be sent directly from the institution to ptoussaint@ph.ucla.edu or hard copies in a sealed envelope may be mailed to the program office.

For transcripts in a language other than English, it is recommended that they either be evaluated by World Education Services or the original transcripts be brought to the Office of Student Services to be photocopied.
