Lockers: Lockers can be found on floors 4 and 6 down at the end of corridor 1 (turn left off of the main FSPH elevators). These are the ONLY lockers managed by the Fielding School of Public Health.
- Complete these steps to reserve a locker:

Bring numbers of preferred lockers on the 4th or 6th floors, student ID, and combination lock to OSS. (Note that key locks are not accepted, as combinations are recorded on the application.)

Read and sign the application form in-person (feel free to request a copy). The person renting the locker must be present to sign the application -- you cannot sign for a friend.

Place your lock on the locker to remove it as an option for other students.

- Graduate students may fill out two locker applications if more than one locker is desired. Lockers are available on a first-come basis. For any questions or concerns regarding student lockers, please contact Kaelyn Duke in the Office of Graduate Student Services.
- Complete these steps to reserve a locker:

Student Lounges: FSPH provides students with two lounges. The Masters Student Lounge is located on A1-240 in the Center for Health Sciences building and the Doctoral Student Lounge is the room door over at A1-250. Doors are unlocked Monday through Friday from 7am to 7pm; just push on the door to go inside. After-hours access requires the use of your Bruin Card. Both rooms have couches, tables, refrigerators, and microwaves. The Masters room also has computers, a conference table, a sink, a lactation room (access code required), and a refrigerator. Students are urged to help keep the rooms clean. The UCLA Fielding Community Space is located on the first floor (16-059).

Public Health Community Space: The new FSPH Public Health Community Space is located on the first floor at CHS 16-059. It is open from 7am-7pm Monday through Friday. Please refrain from eating food or hosting large events in this space; the A-level lounges are to be used for these purposes instead.

PHSA Student Feedback Form
The Public Health Student Association (PHSA) and Office of Student Services (OSS) at the UCLA Fielding School have collaborated to create a form in which students can submit their questions, concerns, ideas, and feedback on how OSS and PHSA can work together to improve the student experience. Please note that this is separate from the formal grievance process. To file a formal grievance, please follow the process listed under “Grievance Process” further down the page.
