Step 1: SOPHAS Application (Required)
Applicants must complete the online application thru SOPHAS (the common application for public health schools). Please submit the following materials to SOPHAS:
- Resume or curriculum vitae (CV)
- Statement of Purpose
- Personal Statement
- Three letters of recommendation — two from former professors and one from an employer. If the applicant has been out of school for more than 5 years, three letters from employers are acceptable.Please note letters of recommendation must be submitted electronically.
- One set of official transcripts from each institution you have attended. International applicants must submit transcripts through World Education Services (WES) for an official WES evaluation.
- Official GRE Test Scores* — UCLA Fielding School of Public Health Code for ETS to report to SOPHAS: 4225 (Please check here to determine if your department is waiving the GRE for Fall 2023 applicants)
- Official TOEFL - Code 5688 SOPHAS in Massachusetts — www.toefl.com. (Click here for English proficiency information)
- Official IELTS scores (for applicants who took the IELTS). Please submit official scores to:

The application priority due date for Fall 2025 is December 1, 2024. Early applications are encouraged to ensure timely receipt of all required documentation.

Applications submitted to SOPHAS and the UCLA Graduate Division after the December 1st deadline may be considered, but will have reduced opportunities for admission, financial support, and housing. If you are applying after December 31, 2024, please check with the department to ensure that it is still accepting applications.

Once all of your application materials have been received and processed you will receive an email notice from the UCLA Fielding School of Public Health’s Office of Student Services that your application is complete and being forwarded to the department for review. If you are missing documents, you will be notified that your application is incomplete with a request for you to forward the necessary documents.
Please note: incomplete applications will not be forwarded to the department for review by the admissions committee.
